Abstract

Denture-supporting oral mucosa is repeatedly exposed to mechanical loading while relying on a terminal microvascular network to restore perfusion between loading cycles. This narrative synthesis examines the vascular behaviour of denture-bearing tissues under local prosthetic loading and systemic circulatory stress. The evidence indicates that sustained compression and cyclic masticatory loading produce different perfusion responses. Sustained loading can markedly suppress mucosal blood flow, whereas intermittent loading may produce net hyperaemia through repeated release and reactive reperfusion. These findings are more coherently interpreted in relation to loading duty cycle and recovery time than to peak interfacial pressure alone. Evidence linking altered mucosal perfusion directly to long-term residual ridge resorption remains limited, with most human studies involving small samples, short observation periods, or surrogate populations. Systemic conditions associated with reduced capillary density or impaired vascular reserve, including diabetes, hypertension, chronic kidney disease, heart failure, and vasospastic disorders, may further constrain recovery after loading, but their interaction with denture-bearing mucosa has not been directly established. Current perfusion-measurement techniques provide useful physiological information but lack sufficient standardisation and interpretability for routine prosthodontic diagnosis. The available evidence therefore supports cautious clinical emphasis on load distribution, tissue recovery, and denture removal during prolonged periods of dependency or critical illness, while highlighting the need for standardised studies of vascular reserve in actual denture wearers.
